On selection, the candidate must enter into an agreement with the Medical Superintendent, ESIC Model & Super Specialty Hospital, Kollam, on non-judicial stamp paper of Rs. 200/-.

The chosen candidate must submit a security deposit of Rs. 15,000/- as a DD in favour of ESIC Account No. 2, payable at Kollam, at the time of joining; this is refundable after successful completion of the contract along with production of a no-due certificate.

Chosen Specialists/Super Specialists must have a professional indemnity policy covering the period of contractual engagement, submitted within 7 days of engagement. Minimum sum assured: Rs. 30 lakh for Anaesthesiologists, Cardiac Surgeons, Neuro Surgeons, Plastic Surgeons, etc.; Rs. 20 lakh for General Surgeons, Gynaecologists, Obstetricians, etc.; in addition to at least Rs. 10 lakh for Physicians, Dentists, Radiologists, Pathologists, etc.
